IMPORTANT NOTES

SPECIAL NOTE FOR WORKERS (Regarding the Visa-Related Medical Examination):

You are here to undergo an embassy-required medical examination as part of your visa application. The examination and tests are done to ensure that no public health, public safety, or unreasonable demand is imposed on the host country.

 

 

THIS IS NOT a POEA-REQUIRED MEDICAL EXAMINATION:

The VISA-RELATED MEDICAL EXAMINATION is different and not accepted by the POEA just as the POEA medical examination is not accepted by the embassy. The embassy’s medical examination is done to protect its citizens. The POEA medical examination is done to protect the OFW.

ANNOUNCEMENT

1. Medical examination is strictly by appointment only.

– No walk in clients will be entertained in the clinic.
– No cash payments will be accepted in the clinic.
– Please deposit your payments to the Bank account posted below in the ‘Payment Options’ section.

2. Be reminded that during this pandemic time, NHSDI Davao is strictly observing social distancing, surgical mask use, face shield use and proper handwashing. You are expected to follow these guidelines once you enter our premises.

 

3. You Will be asked to fill up the ‘symptoms checklist form’ as required by DOH and DTI. Please be honest in filling up this form as this can be a legal matter which concerns public safety. Please do not come for medical check up if you have any symptoms like fever, cough, headache, lbm, sore throat, colds and the likes. If you are not feeling well please stay home and call the designated Numbers of Covid Centers.

 

4. Let us practice the guidelines set by WHO/DOH to stop the transmission of Covid 19. Together we can win this battle.

REMINDERS

REQUIREMENTS TO BRING:
1. Original Passport. No original passport = No medical
2. Referral Letter (HAP ID for Australia or IME ID / IMM1017 for Canada)

*If Upfront Canada and New Zealand No need Referral Letter (HAP/IME#)

3. Brgy. Certificate (proof as not suspected for covid-19 or is not PUM/PUI) ***If from Davao City Area, no need for Brgy. Certificate
4. Deposit Slip (Payment)
5. DTI CHECKLIST (see requirements below)

 

FOR COVID PREVENTION:
a. Waiting area is open air. (tent outside clinic is warm with no electric fan)
b. Chaperon can not sit inside the clinic (except children and elderly).
c. No facemask, No Face shield, No Entry.
d. Observe Social Distancing.
e. Bring your own ball pen and disinfectant.

*If you are positive for covid 19, please call covid-19 center. Once cleared or finished with quarantine protocols, get barangay clearance and a medical certificate from the doctor that you can safely travel and conduct your medical examination.

1. Applicants are advised to take their meals before coming over for their medical examination since food is not allowed within the premises and all initial blood and urine tests do not require fasting.

2. Females within the reproductive age group are advised not to schedule their medicalexamination during their menstrual period. Otherwise, they will be required to come back one week after the last day of their menstruation.

3. For PREGNANT applicants please bring OB-Gyne consent prior to x-ray.

  • Old chest x-ray films not less than 3 months prior to the medical examination
  • Detailed medical certificate regarding a previously-treated or currently managed clinically significant illness which should contain the following information:
    • Complete final diagnosis
    • Duration and date of treatment
    • Surgical procedure or medical intervention done
    • Treatment outcome and prognosis, if available
      • Vaccination Record (must contain the following details):
    • Personal copy (ex: baby book) or a copy of medical chart indicating summary of received vaccines
    • Dates of vaccination
    • Signature of doctor or health worker who administered the vaccine

MEDICAL EXAMINATION FEES:

 

 

Medical Fees quoted are for the basic examinations plus any subsequent necessary investigations and treatment for tuberculosis (TB). Please note that applicants will still have to pay additional fees for any other additional tests/referrals needed for non-TB related issues.

A. Before coming for your medical examination, you must lodge your visa application, either with the Austalian Embassy or via eVisa systems.

REQUIRED MEDICAL FEES

Permanent visa applicants 2-10 years old are required to undertake either a Tuberculin Skin Test (TST) OR a TB Blood Test (Interferon Gamma Release Assay or IGRA).

  • Tuberculin Skin Test (TST) requires 2 clinic visits. Second visit is between 48 and 72 hours from TST placement.
  • The TB Blood Test (Interferon Gamma Release Assay or IGRA) will only require 1 clinic visit if results are negative.

Chest X-ray will be required for children 10 years old and below if they have a positive TST/IGRA result or have exposure to TB Cases or a history of TB disease
