4. PLEASE BRING BLACK BALL PEN, PRINTED AND FILLED OUT NHSDI FORM (please download from our website davao.nhsiphilippines.com), ORIGINAL PASSPORT for Canada visa applicants, and ORIGINAL DEPOSIT SLIP as proof of payment on your appointment date.
