SPECIAL NOTE FOR WORKERS (Regarding the Visa-Related Medical Examination):

You are here to undergo an embassy-required medical examination as part of your visa application. The examination and tests are done to ensure that no public health, public safety, or unreasonable demand is imposed on the host country.

 

 

THIS IS NOT a POEA-REQUIRED MEDICAL EXAMINATION:

The VISA-RELATED MEDICAL EXAMINATION is different and not accepted by the POEA just as the POEA medical examination is not accepted by the embassy. The embassy’s medical examination is done to protect its citizens. The POEA medical examination is done to protect the OFW.

ANNOUNCEMENT

APPOINTMENTS are required as per guidelines by the Local Government.

→ We will start accepting clients through APPOINTMENT BASIS ONLY.

Our clinic is open from Mondays to Fridays 7:30am to 5:00pm, and Saturdays 7:30am to 4:30pm.

→ ALL APPLICANTS 18 years old and below must be accompanied by their parent/guardian

REMINDERS:

1. Applicants are advised to take their meals before coming over for their medical examination since food is not allowed within the premises and all initial blood and urine tests do not require fasting.

2. Females within the reproductive age group are advised not to schedule their medicalexamination during their menstrual period. Otherwise, they will be required to come back one week after the last day of their menstruation.

3. For PREGNANT applicants please bring OB-Gyne consent prior to x-ray.

    • School ID and Passport / Valid ID’s listed above from accompanying parent/guardian/ relative
    • NSO Birth certificate and Passport / Valid ID’s listed above from accompanying parent/guardian/ relative

E. 2 good quality photocopies (preferably in color) of Passport / Valid ID’s clearly showing photo, name and date of birth

F. Please bring black ballpen

G. No fasting required

H. Bring all pertinent medical records, if available

    • Old chest x-ray films not less than 3 months prior to the medical examination
    • Medical report regarding a previously treated or currently managed significant illness which should contain the following:
    • Complete final diagnosis
    • Duration and date of treatment
    • Surgical procedure or medical intervention done
    • Treatment outcome and prognosis, if available
    • Vaccination Record (must contain the following details):
    • Personal copy (ex: baby book) or a copy of medical chart indicating summary of received vaccines
    • Dates of vaccination
    • Signature of doctor or health worker who administered the vaccine

Medical Fees quoted are for the basic examinations plus any subsequent necessary investigations and treatment for tuberculosis (TB). Please note that applicants will still have to pay additional fees for any other additional tests/referrals needed for non-TB related issues.

REMINDERS:

A. Before coming for your medical examination, you must lodge your visa application, either with the Austalian Embassy or via eVisa systems.

B. Applicants are advised to take their meals prior to coming over for their medical examination since food is not allowed within the premises and all the initial blood and urine tests do not require a fasting state.

C. FOR PREGNANT Applicants please bring OB_Gyne consent prior to xray.

D. Females within the reproductive age group are advised not to schedule their medical examination during their menstrual period. Otherwise, they will be required to come back one week after the last day of their menstruation.

E. Applicants who have been scheduled for additional examinations must come on their designated examination date and time.

B. A copy of the Referral letter from the embassy which contains you HAP ID / TRN Number

* If original passport is not available:

  • a Health Examinations List (HAP letter) (or equivalent documentation from the Department that shows which health examinations are required) and a copy of your passport photo page that has been certified by the department or an AVAC.
  • a Health Examinations List (HAP letter) with a photograph attached together with stamp/seal from the Department.
  • For Non-migrating dependent PERMANENT applicants who does not have a passport or already submitted to Australian Embassy, You may present two (2) Government issued IDs:
    • 2 Government issued ID’ s
      • Bureau of Internal Revenue (BIR) ID (laminated with photo)
      • Voter’s ID (laminated with photo)

      • Postal ID

      • (Birth certificates issued by the Local Civil Registrar (LCR) in lieu of NSO Birth Certificates, are not acceptable. For an applicant born in a country other than the Philippines, please bring the birth certificate issued by the government in the country of birth.)

Permanent visa applicants 2-10 years old are required to undertake either a Tuberculin Skin Test (TST) OR a TB Blood Test (Interferon Gamma Release Assay or IGRA).

  • Tuberculin Skin Test (TST) requires 2 clinic visits. Second visit is between 48 and 72 hours from TST placement.
  • The TB Blood Test (Interferon Gamma Release Assay or IGRA) will only require 1 clinic visit if results are negative.

Chest X-ray will be required for children 10 years old and below if they have a positive TST/IGRA result or have exposure to TB Cases or a history of TB disease

6. Minors (18 years old and below) must be accompanied by their parents or any legal guardian who has thorough knowledge on the client’s medical history.
