SPECIAL NOTE FOR WORKERS (Regarding the Visa-Related Medical Examination):
You are here to undergo an embassy-required medical examination as part of your visa application. The examination and tests are done to ensure that no public health, public safety, or unreasonable demand is imposed on the host country.
THIS IS NOT a POEA-REQUIRED MEDICAL EXAMINATION:
The VISA-RELATED MEDICAL EXAMINATION is different and not accepted by the POEA just as the POEA medical examination is not accepted by the embassy. The embassy’s medical examination is done to protect its citizens. The POEA medical examination is done to protect the OFW.
APPOINTMENTS are required as per guidelines by the Local Government.
→ APPOINTMENT HOURS FOR BAGUIO IS AVAILABLE FROM 7:00 AM TO 2:00 PM – MONDAY TO FRIDAY. CLINIC IS CLOSED ON SATURDAY, SUNDAY, AND HOLIDAYS.
→ ALL APPLICANTS 18 years old and below must be accompanied by their parent/guardian
FOR NON BAGUIO RESIDENTS, as per requirements by the Local Government Unit.
1. Register online at hdf.baguio.gov.ph
2. Secure necessary documents (ex. Travel Clearance Certificate) from the Mayor (or his/her authorized representative) of the point of origin.
Upon arrival in Baguio City you may be required to proceed to the triage area before proceeding to our clinic for your medical.
REMINDERS:
1. Applicants are advised to take their meals prior to coming over for their medical examination since food is not allowed within the premises and all the initial blood and urine tests do not require a fasting state.
2. Females within the reproductive age group are advised not to schedule their medical examination during their menstrual period.
3. Applicants who have been scheduled for additional examinations must come on their designated examination date and time.
4. Please bring your prescription spectacles/eyeglasses or contact lenses if applicable.

-Detailed medical certificate regarding a previously-treated or currently managed clinically significant illness which should contain the following information:
• duration and date of treatment
• surgical procedure or medical intervention done
• complete final diagnosis
• outcome of treatment or treatment plans and prognosis, if available
Vaccination Record (must contain the following details):
• personal copy (e.g. baby book)or copy of medical chart indicating received vaccines
• complete date of receipt of vaccination
• must include signature (license number, if applicable) of doctor or health worker who administered vaccine

A Health Examinations List (HAP letter) or equivalent documentation on the department’s letterhead or an email which contains the applicants personal details and lists their required health examination and a photocopy of your passport photo page that has been certified by the department or a contracted Service Delivery Partner (SDP) of the department.
A Health Examinations List (HAP letter) with a photograph of the applicant attached that has been officially stamped by local Australian overseas mission certifying the applicants identity.
